Help! I need a Blood Test Form!!!

Can anyone email me a form (scanned of course) for a blood test. I have been fasting since last night, and went to look for my forms (which I thought were filed) and ... cannot find them!!!! AND>>>my clinic is closed today. I guess I will just have to go through the process all again if I can't get one this morning. Boo hoo.

I know the clinic will be different, but I am sure I can change the details.

Is this your 4 week blood test? Cause if it is you dont need to fast.
